FAC-WF-02 · Corrections, declined agreements, department checklists and withdrawal
The unhappy paths: a reviewer objects, a patient says no, a checklist is about equipment rather than a patient, and a template has to come out of use while someone is half-way through it.
Before you start
Run FAC-WF-01 first: it publishes the medication agreement this journey uses. Same people and patient.
Steps
1. A reviewer asks for changes
On a record waiting for cosignature, a reviewer chooses Ask for changes and says why. The record shows Changes requested. Another reviewer can't cosign over the objection.
2. The nurse corrects it
The nurse sees the reviewer's reason, corrects the answer and completes the step. Earlier signatures stay in the record, marked superseded by a correction; the corrected version needs the patient's and staff signatures again. The answers as first signed are kept.

3. The patient declines
Patient declines or can't sign records Patient declined or Unable to sign with the reason and any follow-up. The record reads Declined — not signed.

4. A department checklist with no patient
The admin uses the Department equipment check starter for General Outpatients. A reviewer who can't (or prefers not to) draw signs with Type name instead: their full name plus the confirmation, recorded as a typed signature.

Staff start it from Department workflows → Start a department checklist. No patient is involved; the list shows it as a Department record.

5. Edit a published template, then withdraw it
Editing a published template saves a new draft revision; the published revision and everything started on it stay as they are. Withdraw published revision stops new starts and asks what to do with records still open on it: put them on hold until a reviewer decides, or cancel them.

6. A held record waits for a decision
The open agreement shows On hold: Template withdrawn: … and can't be filled or signed. A department reviewer either continues it on its original wording or cancels it with a reason.
What Verabase will refuse and why
| You try to… | Verabase says | Why |
|---|---|---|
| Cosign after another reviewer asked for changes | A reviewer asked for changes… | An objection isn't outvoted |
| Reuse signatures after a correction | Signatures marked superseded; new ones required | People sign the exact content |
| Start a department checklist for a patient | This is a department workflow… | No dummy patients |
| Type a name that isn't the signer's | Type the signer's full name exactly… | The typed method still identifies the signer |
| Fill or sign a held record | No current step while on hold | Withdrawn wording isn't silently used |
| Start a withdrawn template | It isn't offered | Withdrawal stops new use |
You're done when…
- [ ] A corrected record shows its earlier signatures as superseded and new ones on the corrected answers.
- [ ] A declined agreement reads Declined — not signed with the reason.
- [ ] A held record was continued or cancelled by a reviewer, with a reason in its history.
Practice task
Withdraw a published checklist with one record still open, choosing put them on hold. As a reviewer, continue it on its original wording. Check: the history names who held and who resumed it, and nobody could start a new one meanwhile.
